For Referrers
Please send us a referral for any neurology consultations via Medical Objects, fax or email. If you wish to discuss any case prior to sending a referral, please do not hesitate to contact us. When referring for a test, please use the NCS or EEG request forms. If your practice uses Best Practice, you can find these forms in your ‘templates’ for your convenience.
For Patients
A referral letter from your general practitioner or specialist is required before an appointment can be confirmed. A referral form can be downloaded for a nerve conduction study or an EEG. Once the referral is triaged, our staff will contact you with confirmation paperwork. You will be requested to provide information and fill in the pre-appointment forms that can be emailed or posted to you. We are really grateful to you for returning these forms to us at least a day before the appointment (via email, fax or post).
Fees
Please contact the reception staff for fees. Consultation generally involves a gap although some items numbers may be bulk billed. A full payment, however, is required on the day. In most instances, we shall be able to submit all Medicare claims for you online.
