Referrals and appointments

A referral letter from the general practitioner or another specialist is required to be faxed or emailed prior to the appointment. A referral form for nerve conduction study can be downloaded here. Dr. Sheikh will review and triage the referral. Reception staff from Dr. Sheikh’s rooms will then contact the patient to notify of the appointment time and any special requirements for the consultation.

In addition to the referral letter, Dr Sheikh pays special attention to the input provided by the patient themselves. To facilitate Dr Sheikh understand your condition better, please download and fill out “My Health – My Voice” form. Please bring it with you at the time of consultation or send it to us beforehand via fax or email.

Please contact the reception staff for fees which would usually involve a gap. A medicare rebate would normal apply that can be claimed on the same day in most instances.
